About this Knowledge Exchange

Coordinated by the SE Health First Nations, Inuit, and Métis team, in partnership with Lakehead University’s Centre for Education and Research on Aging & Health (CERAH), you will have an opportunity to participate in meaningful knowledge exchange with over 150 in-person and 200 virtual participants from across Canada.

Home is foundational to health and well-being. Together, we will work to support a journey home with those living with serious illness within Indigenous communities.

Who Is This Event For?

This national event will unite Indigenous providers, policy professionals, leaders, advocates and allies across Canada to explore strategies and collaborative solutions for advancing Indigenous policy and practice, including:

  • Elders and Knowledge Keepers
  • Health Professionals
  • Policy Professionals
  • Community Health Leaders

Knowledge Exchange Goals

The Knowledge Exchange provides an opportunity to understand the alignment of key initiatives, identify opportunities to address barriers and inform meaningful next steps on the journey home:

1. Indigenous Self-Determination in Palliative and End-of-Life Care.

2. National Engagement on Indigenous Perspectives on Medical Assistance in Dying

3. Wholistic Continuum of Long-Term and Continuing Care

An Opportunity To Connect

One of the greatest advantages of attending this Knowledge Exchange in person is the ability to engage in spontaneous conversations and serendipitous encounters. During networking breaks, meals, and social events, you can strike up conversations with like-minded individuals, exchange ideas, and share experiences. These impromptu interactions often lead to collaborations, partnerships, and lifelong connections that can profoundly influence your professional journey.

Online participants are encouraged to utilize the microphone, camera, chat, and Q&A functions within the online platform application.
